In the medical problems current/past there needs to be a list of specific direct questions with yes no answers which may then link to further questions. Within the consent section there should be the ability to document what written and verbal information has been given. Answers in to the previous questions, patient demographics and op type should drive the ordering of pre op testing, incidently I do not think random glucose appears in the NICE guidance

has there been any trial of this developing work yet? just a basic attempt to time the process? and where is the detail of the ILTC identification protocols? I have been developing protocols for OCC staff for some time, and we have now taken a large number of calls using the protocols, probably 70,000 in the latest version. i have developed them in consultaion with staff and they give a prioity outcome of 13% to triage within 20 mins, and a approx 1-2% to 999 or A and E.However the team have a large number of choices in the questions they ask, which for instance allows them not to ask " are they responding" when it is clear from the initail detail taking that a collapse certainly has not occured.Other options to ask or "skips" allow our experienced call handling team to take the call and prioritise using a fair bit of their own judgment, which in my view has to be allowed so as to keep the process brief In summary I urge an early trial!

At York Hospitals NHS Foundation Trust we have recently developed and are now trailing an electronic Pre Operative Assessment module on our PAS. (We have our own PAS system). How do we share and learn what is being planned? We are in the process of standardising and centralising pre operative assessment as well as planning managing it with on day of admission and waiting list management through one admissions and assessment area for all elective patients in the Trust. The electronic module of CPD (PAS) is key to this standardisation for us.

Initial assessment data set draft 3.2 1/7/07 I think it would be useful to add Tetanus/immunisation Status to section G on Medical History. Rationale - need to know with any wound, cuts, etc.

needs to ensure able to get standard initial assessment at all sites which provide it with no unmarranted variation in outcome depending on site also need to be able to have same triage whether presenting remotely (e.g. phone) or in person
